CONSENT TO PERSONAL DATA PROCESSING

1. I, the personal data subject, confirm that I have read the Personal Data Processing Policy of MODME LLC (MODME), published on the official website https://modme.uz, and give unconditional and explicit consent to MODME LLC (TIN 307442900) to process my personal data in accordance with the laws of the Republic of Uzbekistan and the Contractor's Policy.

2. My personal data is processed for the following purposes:

• registration and provision of access to the modme.uz Service;
• performance of contractual obligations between me (or the organization I represent) and MODME;
• operation of the CRM/LMS platform and related services;
• sending notifications, technical information and messages related to use of the Service;
• compliance with the laws of the Republic of Uzbekistan.

3. The following personal data may be processed under this consent:

• surname, name and patronymic;
• contact phone number;
• e-mail address;
• position and organization name;
• information entered during registration or use of the Service functionality.

4. MODME may collect, record, systematize, store, change, use, anonymize, block, delete and destroy personal data, and transfer it within the Republic of Uzbekistan for the purposes stated in this Consent.

5. Data is processed using automated tools and, where necessary, manually in accordance with the MODME Personal Data Processing Policy and legal requirements.

6. Data is stored on protected servers located in the Republic of Uzbekistan in accordance with the database registration certificate.

7. MODME may transfer personal data to third parties where such transfer is necessary for:

• communication and notification services;
• payment processing;
• hosting and data-center services in the Republic of Uzbekistan;
• performance of contractual obligations;
• other purposes provided by the laws of the Republic of Uzbekistan.

8. I have been informed that I may receive information about the composition and processing methods of my data, request correction, blocking or deletion of data, withdraw consent by sending a notice to [email protected], and appeal MODME actions as provided by law.

9. This consent is valid throughout my use of the Service and for sixty (60) calendar days after relations with MODME end.

10. After this period, data must be deleted or anonymized under the Offer and MODME Personal Data Processing Policy unless otherwise required by law.

11. I confirm that the provided data is accurate and consent to its processing under this document.
